Choose a level of membership, then:

Come by our building at 1411 Asheville Highway,
on U.S. 25 just north of N.C. 191

Mail the completed membership application below with a self-addressed, stamped envelope to: Opportunity House, 1411 Asheville Highway, Hendersonville, NC 28791 -- and we will mail your membership card and receipt.
